Had to remove someone from PT today. Patient who really needs us but can't let go of enough control to allow us to help him. Dude has refused everyone in the dept. I offered to allow him to pick his own treatment time and do whatever he wanted to do with me yesterday and he refused. I went in today and told him he needed us, was worsening, and would not get better until he let us help him. He told me he was doing his PT on his own when we weren't in the room. Since PT doesn't involve lying in bed 24hrs this is incorrect. I educated him on what was happening and what needed to happen and he didn't seem to care. It is sad because he isn't gonna get better but there isn't a way to get thru to him. He acts the same way with the doctors, wound care, and nurses. Refusing their treatments as well. He will probably be the cause of his own death due to flat out not allowing anyone to help him. Crazy folks in this world This may be a person who needs psychological care as well as physical care. When you're being cared for by people who have poured their soul into their work, you listen to their advice.... unless, of course, you think you're smarter than they are. Is this person a covid patient or in for some other reason?

Is this person a covid patient or in for some other reason? He's a regular patient. History of 3rd degree burns and a stroke. Sad part is he could get up and walk around with me and be fine. He will be discharged to a long term care facility--that disqualifies you from PT or getting better. You lay in a bed and rot. That's by HIS choice.
It's all psych Ken. He has to try to control every person who walks in the door. His only control method is to refuse anything that's suggested. I recognized it yesterday during my first attempt at treating him. I read all the progress notes and it was like this:
DR: Patient complained about leg pain. I stated that we needed to do a CT on it. Patient stated he didn't think that was necessary, refused the CT.
Wound care: Pt questioned why we weren't putting medicated bandaging on his healed burns. I told him they were completely healed and then he demanded the bandaging. I re-stated they were not necessary. Pt stated "You just wait until Friday and look at them then. See what happens!"
Here's what's bad about this situation and why I had his PT orders removed. He won't work with us due to his mental lock down and inability to see that we are trying to help him. I can keep trying daily but all it does is take valuable time away from my patients who want to get better. It's best just to pull back and let the shlt hit the fan and hope it kicks his brain into survival mode. Hopefully it happens in the hospital and he doesn't actually get to long term care. Removing him from PT will let him stew on it without being in defense mode any time we walk in. Or not...
